Services

Community Case Management

Coordinated Case Management includes advocacy, linkage and referral to other needed services and resources.

Consultive Psychiatric Services

In conjunction with treatment services, Cornerstone offers the services of board-certified psychiatrists and psychiatric mental health nurse practitioners. This treatment includes a psychiatric evaluation and follow-up medication management, if needed.

Cornerstone Friends and Family Group

This group is open to parents, siblings, and other family members of an individual who receives services at Cornerstone. This is intended to teach about the agency and various resources available and answer questions about state and federal benefits. The Parents and Family Group is an opportunity for family members and guardians to engage with the agency, learn about important issues and events, and socialize with other families. Meetings are usually held four times a year. To find out about the next meeting, contact Cornerstone at 815-741-7602.

Mental Health and Substance Use Services

We provide a wide array of mental health and substance use support services. Some of the evidence-based practices Cornerstone provides include: Individual Placement and Support (IPS) employment services, Wellness Recovery and Action Plan (WRAP) 12-week workshop, Harm Reduction, Seeking Safety and other trauma-informed services, and Mindfulness-Based Cognitive Therapy. Housing programs for individuals experiencing homelessness are available for those who may qualify for one of our grant funded programs.

Therapy Services

Outpatient Therapy provides adults who have a diagnosed mental health condition or co-occurring substance use disorder with therapy on a scheduled and ongoing basis. As a consumer of these services, clients will be able to meet with a qualified therapist on-site for individual and/or group therapy services. Sessions may address general symptoms or a specific problem such as low self-esteem, poor impulse control, depression, or unresolved trauma and grief. Clients may also have a case manager assigned to them.
